Love the idea that Simplicity is publishing adaptive patterns. I’ve altered ready-t0-wear clothing for friends who have a cast one leg that make it hard or impossible to put pants on. For underpants, cut the seam open on the side where the cast is, sew some non-stretch fabric on both sides of the seam opening and stitch on snaps (at least 1/4 inch diameter) to close the reworked seam. To adapt sweat pants do something similar, but be sure to move tie from the front of the pants to the side. If you use snaps to close the legs, be sure to put a non-stretch fabric under the sweat pants fabric. Attach snaps using a mechanical snapper.
